Privacy Policy

This Privacy Policy explains how Revamend ("Revamend", "we", "us") collects, uses, and protects information when you visit revamend.com, complete our online onboarding, or use our denial recovery services. Revamend provides claim denial recovery services to U.S. dental practices. We handle protected health information (PHI) only as a business associate of our practice clients, under HIPAA and an executed Business Associate Agreement (BAA).
Information we collect
We collect three categories of information:
- Contact and practice information. When you start onboarding or contact us, we collect the practice name, your name and role, email address, phone number, state, practice management system, and claim volume estimates.
- Remittance and claims data. Files you upload during onboarding or share with us during the engagement, including ERA (X12 835) files, denied claim exports, and supporting documentation. These files contain PHI such as patient names, dates of service, procedure codes, and payment details. We handle this data exclusively under HIPAA and the BAA between Revamend and your practice.
- Website analytics. Standard usage data such as pages viewed, referring site, browser type, and approximate location derived from IP address. We use this to understand how the site is used and to improve it. Analytics data is not combined with PHI.
How we use information
- To provide denial recovery services: triaging denials, preparing appeals and corrected claims, communicating with payers, and reporting results to your practice.
- To operate, secure, and improve our website, portal, and services.
- To communicate with you about your engagement, invoices, and service updates.
- To meet legal, regulatory, and contractual obligations, including obligations under HIPAA.
We use PHI only as permitted by the BAA: to perform the services, for our proper management and administration, and as required by law. We do not use PHI for marketing.
We do not sell your data
We do not sell, rent, or trade personal information or PHI to anyone, for any purpose. We do not share client data with third parties except with the subprocessors described below, or when required by law.
Subprocessors
We use a small number of infrastructure providers to deliver the service. Each provider that may touch PHI is bound by a business associate agreement or equivalent contractual safeguards:
- Cloud hosting provider: application hosting and file storage, encrypted at rest.
- Database provider: managed database services for claim and engagement records.

Security
We maintain administrative, physical, and technical safeguards appropriate to the sensitivity of the data we handle. Data is encrypted in transit and at rest, access is limited to personnel who need it to perform the services, and access is logged. No method of transmission or storage is perfectly secure, but we work to protect your information consistent with HIPAA's Security Rule.
Retention
We retain practice contact information for the duration of the engagement and as needed for legal and accounting purposes. We retain remittance files and claims data only as long as necessary to perform the services and meet the record-keeping obligations in the BAA and MSA. At termination of the engagement, PHI is returned or destroyed as described in the BAA.
Your rights and choices
- You may request access to, correction of, or deletion of the contact information we hold about you.
- You may opt out of marketing emails at any time using the unsubscribe link or by emailing us.
- Patients should direct requests about their health information to their dental practice, which remains the covered entity and record holder. We support our practice clients in fulfilling those requests as the BAA requires.
Changes to this policy
We may update this policy from time to time. We will post the revised version here with an updated date, and we will notify active clients of material changes by email.
